How often do you need have a poo?
How long does it usually take?
What if any activities do you partake in while sitting on the toilet?
I usually start having my poo before/just after/way after i sit on the toilet
Do you pee while you poop
Do you fidget while you poop
If you need to poo do you go right away or hold it
How far do you put your pants down
Do you sit bend or squat while pooing
Do you sit or stand to wipe
Did you ever poo outside
Did you ever poo On a tree []
In the trash can []

If I am outdoors my preferred position to go is: (choose 2 maximum)

How many people live or regularly stay with you?
I allow someone in my household to (shower, bathe, dress etc.) in the same bathroom while I poo
When I need to poo I: (check all that apply)

usually undo or start to undo my clothes to go outside the stall/washroom[]
