A while ago I was at a classmate's house and we were playing PS3. His little sister kept coming in the room wanting to play fight. He kept chasing her away and well 1 time he stepped out and she came in the room and she grabbed my "stuff". I feel bad because this is his little sister (11yo) so I left when came back in the room I haven't been back and he keeps asking me when am I coming over again, but I dont wanna tell him what happen with his sister cuz he might get mad plus I was uncomfortable because she's the same age as my sister, what should I do should I tell him?

You did nothing wrong, his sister has probable mental health issues and there's no issue here - don't let your friendship with whoever this guy is be affected by some eleven year old girl who's been watching the Discovery Channel for too long.

Did his sister do it intentional? If so, I would tell him but not in a insulting way like "Dude I think you sister has a thing for me" but do it like " Hey im not sure if (Sister) did it by accident or not but she touched my junk by accident and I've just been feeling a bit uncomfortable." I think that you should just tell him to get it off of your chest and then you will feel better. So you dont want to ruin your friendship with your good friend.

Since his sister is 11, I really wouldn't worry about it all too much. It's very likely that the only reason she grabbed you there was to hurt you during fighting, not to pleasure you (or anything much else for that matter).
Your friend probably won't really care about the event, but if you keep it to yourself for longer, he'll think you've read more into it and get mad because of that. You might as well just tell him - he's extremely unlikely to fall out with you over something like this.

I guess I will look at it as if it wasnt done on purpose, she didnt pull away like she didnt mean to do it. But at 11 I guess she couldnt know what she was doing

You should tell your friend what happened because he will begin to think that you hate him.

Well I told him and he just said that she likes attention and he will tell her not to bother us the next time I come over
