For me, at least, it started small, with fingernails. Maybe even before that, as when I was little I would devout time to picking at scabs or at the edges of my fingers.
It grows on you, gets bigger until you need to do more and more to get it to feel right. I don't think most people start with a cut down to the fat. Self-harm creeps up on you until you're too far in to stop. Maybe you don't even want to. I know I never thought I'd get all that deep.
Besides, it does
help in the short-term, even if it's possibly harmful long-term. Most people don't think much about long-term consequences, though.
I don't think you can really understand it completely unless you do it yourself.
